Two weeks following the events of Arkham City, Harley injected a thug from her newly-formed gang with Titan to fight Robin when he infiltrated the Shipyard to rescue Batman. A Titan crate could also be seen as apart of the giant Joker monument, and was used to build his arm, which implied that more remnants of Titan remained. At some point, the Joker was the cause of an incident in which a lot of people died, and while Batman and Robin tried to stop him, he managed to get away. [2]While Bane was able to defeat Penguin's TITAN Henchman, he was forced to retreat by Solomon Grundy. In Batman's nightmare of Joker possessing his mind and murdering all of Gotham's villains, Only You can be heard playing through an old record player. This disease was possibly inspired by Batman Beyond: Return of the Joker in which DCAU Joker took over Tim Drake's body using Project Cadmus technology to copy his mind and implant it within Tim following torture sessions that turned him into a surrogate son named Joker Jr. Noting that all the formula needed was a specific enzyme that could be found in the blood of Ra's al Ghul (due to his constant usage of the Lazarus Pit), Batman managed to locate the League of Assassins' stronghold underneath Arkham City in Wonder City, and obtained a sample of Ra's blood. Returning the sample to Freeze, Harley Quinn managed to steal the developed cure as Freeze was attempting to use it as a bargaining chip to force Batman to find his wife, Nora.

All of this made him an incredible dangerous individual even the most powerful superheroes are afraid of. The Joker is one of the two villains of the saga to appear in all four titles and play an antagonistic role against Batman, the other being the Riddler.
When a normal henchman was injected with the TITAN Formula, the henchman mutated, and became insane. However, when the Joker injected himself with TITAN, even though he had mutated, he kept his personality. That could very well be because the Joker was already insane. Titan Joker is a giant monstrous version of the Joker and the final boss in Batman: Arkham Asylum. Titan Joker is about as tall as Killer Croc. He's an incredibly muscular monster, a lot of his bones such as a large amount of his ribs on both sides as well as his fingernails are sharpen and revealed, and his hair is now a mohawk. DEAD!Meanwhile, Joker used the isolated strain of Titan as a bargaining chip, and had his blood that was filled with the Titan Disease sent all over Gotham and injected into Batman. In the early development, the Titan Henchman's transforming structure was with bursting metal from its skin. Later it was replaced with bone structure in the final rendering.
