A. General

You won't be able to get a prescription for more than 4 months' supply at a time because this is its shelf life. When you've taken the ring out, you don't put a new one in for 7 days (1 week). This is the ring-free interval. You might have a period-type bleed during this time. For the treatment of postmenopausal symptoms, Hormone Replacement Therapy (HRT) should only be initiated for symptoms that adversely affect quality of life. In all cases, a careful appraisal of the risks and benefits should be undertaken at least annually and HRT should only be continued as long as the benefit outweighs the risk. Women already on chronic anticoagulant treatment require careful consideration of the benefit risk of use of HRT.

I. Geriatric Use

After the ring has been in your vagina for 21 days (3 weeks), you remove it. This should be on the same day of the week that you put it in. To remove the ring: you may need emergency contraception if you've had sex since putting in the new ring or during the ring-free break This medication can increase the risk of developing a condition (endometrial hyperplasia) that may lead to cancer of the lining of the uterus. Taking progestins, another hormone medication, with this medication lowers the risk of developing this condition. Therefore, if your uterus has not been removed (by a hysterectomy), your care team may prescribe a progestin for you to take together with your estrogen. You should know, however, that taking estrogens with progestins may have additional health risks. You should discuss the use of estrogens and progestins with your care team to determine the benefits and risks for you. In a Phase I study of 14 postmenopausal women, the insertion of ESTRING (estradiol vaginal ring) rapidly increased serum estradiol (E 2) levels. Peak serum estradiol concentrations post-initial burst declined rapidly over the next 24 hours and were virtually indistinguishable from the baseline mean (range: 5 to 22 pg/mL). Serum levels of estradiol and estrone (E 1) over the following 12 weeks during which the ring was maintained in the vaginal vault remained relatively unchanged (see Table 1).

Tell your doctor if you just had or will be having surgery, or if you will be confined to a chair or bed for a long time (such as a long plane flight). You may choose to put the insert into your vagina using the lying down or standing up position. Put the insert about 2 inches into your vagina, with the smaller end up, using your finger. Use the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not use extra medicine to make up the missed dose.
