Red grapes are well-known for their beneficial antioxidants, and red vine leaf extracts are derived from the leaves of the same grapevine, Vitis vinifera. Red vine leaves are harvested when they achieve a rich red colour and have the highest content of antioxidant polyphenols. Their extracts are used medicinally for their circulatory effects, especially within leg veins. They reduce inflammation, stabilise the endothelial lining of capillaries and veins, and promote their repair to reduce swelling of the ankles. Red vine leaf benefits After 3 months of treatment, those who also received the red vine leaf extracts showed greater improvements in blood clotting factors and in the dissolution of the fibrin clot, with a more pronounced opening up (recanalization) of veins in the affected area. In a study involving 30 people with acute deep vein thrombosis, all received usual medical treatment with anticoagulants, and half were also given red grape leaf extracts. Of the 260 patients enrolled and randomized, 219 completed the study in accordance with the protocol. In the intention-to-treat analysis (N = 257), the mean (+/- SD) lower leg volume (measured by water displacement plethysmography) of the patients treated with placebo (N = 87) increased by 15.2 +/- 90.1 g (displaced water mass) and by 33.7 +/- 96.1 g after 6 and 12 weeks compared to baseline. In contrast, for patients treated with AS 195, lower leg volume decreased, and after 12 weeks of treatment, the difference in mean lower leg volume between the active treatment groups and the placebo group was -75.9 g (95% CI: -106.1 to -45.8 g) and -99.9 g (95% CI: -130.3 to -69.6 g) for the group treated with 360-mg AS 195 (N = 86) and 720-mg AS 195 (N = 84), respectively. The changes in calf circumference showed a similar pattern: in patients treated with AS 195, both the higher dose (720 mg) and, albeit to a lesser extent, the lower dose (360 mg) resulted in a clear reduction in circumference over time, whereas, circumference remained largely unchanged in patients treated with the placebo (95% CI of the estimated treatment effects vs. placebo after 12 weeks: -1.40 to -0.56 cm and -1.73 to -0.88 cm for 360 and 720 mg AS 195, respectively). These differences were statistically significant (p < 0.001). The reductions in ankle circumference were qualitatively similar but quantitatively less marked. Subjectively, there was an improvement in key CVI symptoms (VAS) at 6 weeks with all treatments, but a further improvement at week 12 was seen only in the active treatment groups; at 12 weeks, the changes compared to baseline were significantly greater (p < 0.001) in both active treatment groups than in the placebo group. The treatments were well tolerated; Adverse events were rare and usually mild. Two adverse events (AEs) during treatment with the placebo led to hospitalization and were hence labeled as 'serious'. One of the first studies to assess the effectiveness of red vine leaf extracts (Antistax) asked 71 people with leg swelling (due to chronic venous insufficiency) to take 360mg red vine leaf extracts, or placebo during two separate periods of 6 weeks. This meant that each person tried both treatments and acted as their own controls.In Germany, as many as 7 % of the general population take red vine leaf extracts (Antistax), usually on the advice of their doctor, to reduce swelling and other leg symptoms associated with chronic venous insufficiency. A systematic literature search was performed to identify trials that reported the impact of red vine leaf extract on CVI. The primary outcomes investigated were Leg (limb) volume, calf circumference, ankle circumference, tired and heavy legs, a sensation of tension, tingling sensation, and pain. Red vine leaf extract had a beneficial therapeutic role in patients with CVI. Further high-quality trials are required to be carried out to provide strong evidence. From the 56 studies, 5 trials were selected according to our inclusion criteria. Red vine leaf extract significantly improved numbers of outcomes (lower leg volume, calf and ankle circumference, tired, heavy legs, tingling sensation, pain, the sensation of tension in the legs, cutaneous microcirculation, and transcutaneous oxygen pressure) in only some trials. This systematic review was aimed to assess the effects of AS 195 in patients with CVI.Red vine leaf extract (RVLE) is a herbal medicine containing several flavonoids, with quercetin-3-O-beta-glucuronide and isoquercitrin (quercetin-3-O-beta-glycoside) as the main components. Red vine leaf extracts stabilise the endothelial lining of tiny capillaries and promote their repair and reduce their permeability. Study criteria were evaluated at baseline, after 6 and 12 weeks of treatment and 2 weeks after discontinuation of treatment. If the leg vein valves collapse, then swollen veins become distended and tortuous and may bulge beneath the skin to form varicose veins. This was followed by another study, involving 248 patients with more severe chronic venous insufficiency, classed as moderate-to-severe. Half took 720 mg red vine leaf extract per day for 12 weeks, while half took inactive placebo. The red vine leaf extract significantly reduced lower limb volume by 20 ml more than placebo (27ml versus 7ml) with an associated decrease in leg pain.
